General Terms and Conditions
Specialized Autism Services
Providing Support to Children & Families affected by Autism
It is mutually agreed between the Parties (the “Client” which shall include the “Family”, the “Parent” and or the “Caregiver” and Specialized Autism Services (also referred to as “SAS”) as follows:
Article 1-Session Cancellation Policy
The Client agrees that in the event they must cancel any type of session with SAS, such cancellation and notification must be received by SAS at least 24 hours prior to the scheduled session via telephone and/or email. Otherwise, the Client agrees to pay the full amount of the originally scheduled appointment fee.
If an SAS employee arrives at the scheduled appointment and deems the child too sick to work and consequently cancels the appointment, the Client agrees to pay the full amount of the scheduled session.
Repeated cancellations by the client will result in the cancellation of services provided by SAS, no refunds will be given to the client unless otherwise agreed upon by SAS in writing via email.
Article 2-Cancellation of Services Policy
In the event that the Client chooses to cancel the agreement of service provided by SAS, the Client agrees to give SAS 3 month’s prior notice in writing (via email or on paper). The Client also agrees to pay all outstanding and future invoices which were accumulated and agreed upon prior to ending the said agreement.
Article 3-Confidentiality and Duty Not to Discuss or Disclose Certain Information
SAS acknowledges that during the term of this Agreement, it may have access to and become acquainted with confidential information about the Child, the Client and/or the Child or Client’s family. SAS agrees that it will not discuss or disclose any information of a confidential nature, directly or indirectly, or use any such information with any third party during the term of this Agreement or at anytime thereafter, except as required by law, (such as in cases of child neglect or abuse) unless it obtains the prior written consent of the Client. The Client agrees not to disclose any fee or contract information to any other person and agrees to keep all discussions confidential.
Article 4-Duty to Report any Child Abuse or Neglect
The Client understands and agrees that SAS, and its employees are required to and shall immediately report and/or notify any governmental agency of any and all indications of child abuse and/or neglect observed or detected before, during or after the course of any session with the Client's child.
Article 5-Videotape
SAS does not permit clients or parents to videotape or in any way record intervention sessions unless prior written permission has been given by SAS.
Article 6-Observations by Outside Agencies/School Personnel
Outside individuals that are involved in the Client’s program (such as the child’s teacher, speech therapist, occupational therapist, etc.) may observe an SAS session only if the SAS supervisor is present. SAS requests that the parent contact SAS at least one week in advance to request their presence at the session during which an outside individual wishes to observe.
Article 7-Schedules
SAS will try to accommodate the Client’s scheduling needs as much as possible. The Client agrees to call SAS and send an email to make all requests for schedule changes or changes in location of intervention at least 24 hours in advance otherwise the Client agrees to pay for the full amount of the originally scheduled session. Schedules should be agreed upon at least 6 months in advance.
Article 9-Closure for Holidays
SAS will be closed in observance of the following holidays: Christmas Day (December 25th) and New Year’s Day (January 1st). SAS may also be closed for one week in December, between Christmas and New Year’s Eve. This will be determined in November and Clients will be informed about this possible closure at the time. No service will be provided on these days. The Client will also be notified via email for all other closures for other holidays, training sessions, personal time off, and sick days throughout the year.
Article 10-Client Satisfaction
SAS welcomes any feedback that the Client has to offer regarding any aspect of SAS's services. SAS requests that all feedback be directed to the owner of the company.
Article 11-Responsibility for those not employed by SAS
The Client understands and agrees that SAS is not responsible for the actions or behavior on the part of tutors, therapists or others hired by the Client.
I have read, understood and agree to all of the above statements included in the Specialized Autism Services General Terms and Conditions document (3 pages). This document is sent via email to all clients and applies to all current and past clients of SAS.
SAS reserves the right to amend these terms. It is the client’s responsibility to refer back to the website for any and all updates to this document.
